New Patient Registration

registrationWe welcome new patients who live within our *Practice Area. Registration documents can be obtained on the links below, or can be collected from Reception.

To register you will need to complete all the forms and then bring them to Reception with your identification.

Acceptable forms of identification include

  1. Passport
  2. Photographic driving licence 
  3. Birth /marriage Certificate 
  4. Bank/building society cards/statements
  5. National Insurance number
  6. Medical card
  7. Payslip
  8. Letter from benefits agency
  9. Home office papers
  10. P45

 Confirmation of current address (ie within our area) is also required

  1. Utility Bill
  2. Council Tax Bill
  3. Tax Notification
  4. Bank Statement
  5. Home or Motor Insurance Certificate

At Reception you will be invited to attend a 20-minute new patient medical.  Your registration will not be accepted unless you attend the medical.  (If you are housebound or have other access issues please speak to us about alternative arrangements).

*In January 2015 GP Practices were given the opportunity to register patients who live outside the Practice area.  This is a voluntary arrangement which Bewick Crescent Surgery has not signed up to.
